       Due to the high cost of the hand made glass and plastic samples in our kits, we try provide small kits for
       free to specifying architects and designers only, and we request that designers provide shipping account        numbers like Fed Ex, in order to share sampling costs. Please fill in your shipping acct number in the cell
       below (Fed Ex, UPS, DHL - FedEx preferred), or you can pay by credit card (fax CC# to 440-888-3322,
       charged for shipping only). Large sample binders are available that cost JBD $350.00 each, but charge
       to you only $95.00.

       Some larger samples up to 4" x 6" of specific products that can be sent no charge, but any samples at
       12" x 12" or larger will be invoiced to customer, with the possibility of deducting that cost off of first order.
